My sister often say that her son (my nephew) (15y) is too ugly and way less attractive than his older brother.

Sometimes she joke about him not being her real son too (the nurse gave her the wrong baby)

But besides that she love him.

I honestly dont know how to react with this

Being silent is worst. You have to step up and tell you sister the only ugly thing that you see is her and her behavior.
